# Medication Chart
If your services involve administering medication (e.g., Nursing), you can use the **Medication Chart** to track staff activity efficiently on Comm.care.
Follow the steps below to set it up:
# Step 1: Create Medication Chart Template

To create a reusable **Medication Chart template**, follow these steps:
(1) Go to **Organisation > Templates > Add Template**
(2) Enter the following details:
- **Template Name:** e.g., _Medication Checklist_
- **Template For:** Select Notes (this means the template will be used for Notes in the client’s Circle).
- **Description:** Enter a brief description of the template’s purpose.
- **Document Title:** e.g., _Medication Checklist Week FROM - TO_
- **Document Description:**
- Medication Chart: Depending on your organisation’s requirements, a standard format of the medication chart table might include:
|**Date & Time**|**Medication Name**|**Dosage**|**Method (Oral, Webster Pack, Eye Drops, etc.)**|**Type (P, Ad, As, DNO)**|**Performed 9 Rights (Y/N)**|**Staff Name**|

# Step 2: Create Circle Note with the Template

Once your **Medication Chart template** is ready, you can apply it to a **Circle Note**.
As a manager, it is recommended for you to create one note for every week.
To do this:
(1) Go to **Circle > Timeline > Add Document > Note > Medication Checklist**.
(2) Edit the document title by entering the start and end dates for the week.
(3) Save changes.

_As a manager, it’s recommended to create a new Note for each week._
# Step 3: **Update the Medication Chart**
Once the **Medication Chart Note** is created:
- Case workers should **open the Note and log medication details** each time they administer medication.
- You can **monitor updates** throughout the week to ensure staff enter data correctly.
- By the end of the week, you’ll have a **complete record** of medication administration for that client.
- For the next week: Simply **repeat Step 2** to create a new **Medication Chart Note**.
